BMI

Body mass index (BMI) is the relationship of a person's height and weight. Candidates for obesity treatment include Detroit, Troy and Rochester residents who have a BMI of 35 to 40 and have one of five co-morbidities. Also, a BMI greater than 40 or are 100 pounds (45kg) above their ideal weight.

New criteria for qualification for Bariatric surgery by BCBS 2011 Federal Employee bariatric policy update:

  • 18 years or older with two years of morbid obesity diagnosis prior to surgery
  • BMI 40 or more
  • BMI 35 or more with co-morbidities
  • Participation in medically supervised program for 3 months prior to surgery
  • Nutritional Assessment
  • Psych Assessment
  • Patient has not been treated for substance abuse for 1 year prior to surgery
  • Patient has not smoked for 6 months prior to surgery
  • Evidence that attempts at weight loss in the 1 year period prior to surgery have been ineffective. Prior Auth/ “Pre-Service Review” must be submitted with the following info:

    1. Patient and provider info
    2. Location services are rendered
    3. LOMN
    4. CPT/HCPS codes
    5. Any supporting docs to include test results, etc
  • Estimated turnaround time for “pre-service review” is 21 days
